Registration for Youth Initiative 2010
1. Download the Registration Form. (You may need to install
Adobe Reader if you do not already have it.)
2. Fill out the Registration Form in its entirety, answer the Short
Answer Questions, and get all signatures required.
3. Mail/Fax the Registration Form, Short Answer Questions,
Liability Release, and $250 registration fee to:

4. All Registration Forms must be postmarked no later than
May 20th, 2010.
* Checks should be made out to "Youth Initiative"

Who Can Register for YI?
1. Each church may send up to five students plus one chaperone / youth worker.
2. Rising 10th - 12th grade students are welcome to register. Students must have completed the 9th grade.
Recently graduated seniors are welcome to attend.
